Biomechanical and Histological Evaluation of an Allograft Anchor for Pedicle Screw Augmentation in an Ovine Model

چکیده
INTRODUCTION Achieving solid implant fixation to osteoporotic bone presents a challenge to both spinal surgeons and hardware designers. Loss of surgical construct stability as a result of screw loosening is a well-known complication [1,2], particularly in patients with poor bone quality [3,4]. Thus, the development of novel strategies to address the issue of obtaining enhanced bone-to-screw purchase to achieve necessary construct integrity is an important issue. In the current work, we investigated the effect of lumbar pedicle screws augmented with a bone allograft anchor in an ovine model with temporal biomechanical testing and histological analyses. We hypothesized that the allograft sheath would be gradually incorporated by the host, thereby promoting enhanced pedicle screw purchase.

Comparison of Expansive Pedicle Screw and Polymethylmethacrylate-Augmented Pedicle Screw in Osteoporotic Sheep Lumbar Vertebrae: Biomechanical and Interfacial Evaluations
BACKGROUND It was reported that expansive pedicle screw (EPS) and polymethylmethacrylate-augmented pedicle screw (PMMA-PS) could be used to increase screw stability in osteoporosis. However, there are no studies comparing the two kinds of screws in vivo. Thus, we aimed to compare biomechanical and interfacial performances of EPS and PMMA-PS in osteoporotic sheep spine. METHODOLOGY/PRINCIPAL F...

Study Design Mechanical study. Purpose To compare the pullout strength of different screw designs and augmentation techniques in an osteoporotic bone model. Overview of Literature Adequate bone screw pullout strength is a common problem among osteoporotic patients. Various screw designs and augmentation techniques have been developed to improve the biomechanical characteristics of the bone-...
متن کاملA Biomechanical Comparison of Expansive Pedicle Screws for Severe Osteoporosis: The Effects of Screw Design and Cement Augmentation
Expansive pedicle screws significantly improve fixation strength in osteoporotic spines. However, the previous literature does not adequately address the effects of the number of lengthwise slits and the extent of screw expansion on the strength of the bone/screw interface when expansive screws are used with or without cement augmentation. Herein, four designs for expansive pedicle screws with ...
متن کاملRestoration of pull-out strength of the failed pedicle screw: biomechanical comparison of calcium sulfate vs polymethylmethacrylate augmentation.
OBJECTIVE The aim of the present study was to compare calcium sulfate (CAS) and polymethylmethacrylate (PMMA) bone cements used for the augmentation of a failed pedicle screw with biomechanical pull-out strength (POS) testing. METHODS Thirty lumbar vertebrae were harvested from 6 calves and bone mineral densities (BMD) were measured. Primary polyaxial pedicle screws were randomly inserted and...
متن کاملBiomechanical Comparison Between Bashti Bone Plug Technique and Biodegradable Screw for Fixation of Grafts in Ligament surgery
Background: Ligament reconstruction is a common procedure in orthopedic surgery. Although several popular techniques are currently in use, new methods are proposed for secure fixation of the tendon graft into the bone tunnel. Purposes: We sought to introduce our new technique of Bashti bone plug for fixation of soft tissue graft in anterior cruciate ligament (ACL) reconstruction and to compar...
